Building your CE-Gen™ Course Evaluation is fast and easy.

First, there is no obligation.  This is a free service provided by FacultyEvaluation.Org. 

A PDF of an 8 1/2" x 11"- 2 sided form will be created that can be scanned on any optical scanning machine or Scantron type scanner.  You simply have to print and distribute the form in your classrooms.  Click here to see a sample

Second, you can create as many forms or variations as you require.

Third, the completed PDF will be sent to you in an email.
